Özet

Acute myocardial infarction (AMI) is a rare complication of infective endocarditis. We report on a 48-years old man with acute non Q-vave myocardial infarction due to bacterial aortic valve endocarditis. The patient was admitted with high fever (up to 39 degree C), hypotension (90/60 mmHg) and tachycardia (110 per minute) in our clinic. Multiple blood cultures demonstrated alfa-heamolytic streptococcus. Transthoracic echocardiography disclosed mobil vegetation at the aortic valve. Therapy was begun with penicilline G, 24 million unit per day and gentamycin, 160 mg per day. Eleven day later, he suddenly developed chest pain. Electrocardiography was consistent with acute non Q wave myocardial infarction with ST segment depression in anterolateral leads. Biochemical evaluation supported this diagnosis. Two-days later, the patients died of ventricular arrythmia. This is the first reported case of AMI caused by infective endocarditis in our clinic.
