Özet

The effects of clonidine premedication on sedation, anxiety, haemodynamic response to induction and laryngoscopy, intravenous anaesthetic dose, plasma cortisol and glucose levels and recovery time were compared with diazepam in patients. Study groups consisted of ASA I-II class 50 patients undergoing abdominal hysterectomy. 60-90 minutes before surgery, patients were given oral clonidine 150 μg and famotidine 40 mg in Group Clonidine and oral diazepam 10 mg together with famotidine 40 mg in Group Diazepam. Mean arterial pressures (MAP), heart rates (HR) were recorded at the before premedication, after induction, after intubation, while extubating and at the postoperative period. Anxiety level was assessed using Lineer Anxiety Score (LAS) and sedation status using a three point scale was assessed at the before premedication and before induction. Quality of induction was assessed after intubation. Post anesthesia recovery time was recorded. After the intubation; HR was not changed in the Clonidine Group but was increased in the Diazepam Group (p<0.05). While extubating and postoperative 30. minute, HR was significiantly increased in Diazepam Group than Clonidine Group (p<0.05). MAP were significiantly decreased before induction, intraoperative 60. minute and discharching the recovery room in the Clonidine Group than Diazepam Group. Clonidine produced more sedation and less anxiety. In the Clonidine Group; 2 min. after the intubation, plasma cortisol level decreased (p<0.05), quality of induction was higher and recovery time was shorter than Diazepam Group (p<0.05). 150 μg clonidine provided better anxiolytic and sedative effects, attenuated stress response and incre-ased the quality of anesthesia than 10 mg diazepam at the preoperative medication.
